Financial Counselors
The financial counselors are specially trained in transplant financial issues. They take referrals, check benefits and help case managers, patients and families through our process.

Pre-Transplant Process:

• The patient is assigned a pre-transplant coordinator and a new patient packet is sent to the patient. Once the packet is returned to the pre-transplant coordinator, the evaluation process will be scheduled

Post-Transplant Process:

• After transplant a post-transplant coordinator will be assigned.
• Following a kidney and/or pancreas transplant, the continuum of care will be coordinated by the post-transplant coordinator. This will require consistent communication between the transplant physicians, the patient, the referring doctor and insurance case manager.

Donor Information:
A dedicated donor coordinator is available to provide support and education to donors. They will help donors to be assessed for this wonderful gift while ensuring that their health is maintained to the highest degree.

The Liver and Intestinal programs have adult and pediatric transplant coordinators that are responsible for liver, intestinal and other combination transplants with liver. These coordinators rotate between inpatient and outpatient services.  Please request to speak to an adult or pediatric coordinator for either inpatient or outpatient.

Heart

Many of our patients are referred from a heart failure evaluation. These calls can be referred to either the financial counselors or the transplant coordinators.

Nurse Insurance Coordinator
The nurse insurance coordinator provides you with a contact that will help assist in getting you a plan of care, protocols, results from evaluations, etc. If you have further questions about the clinical picture, they will assist you in contacting the clinical coordinator that has been assigned to your patient by Disease Management.
